Client Forms

New clients will need to fill out some forms before therapy can begin. To make the process as smooth and efficient as possible, the most important documents are provided as downloads right here on my website.

Simply click on a form to download, print, complete and return the documents to me at our next appointment.  All forms are also available at my office, however, filling them out in advance saves time.

My fee is $150.00 per hour.  I can bill most insurance as either an in-network or out-of-network provider.  Please consult your particular benefits plan if you are interested in using insurance.  I am currently in-network with the following carriers:

Aetna

Cigna

United Health Care/UBC

Oscar Health

Oxford Health

If you prefer to pay privately, I am also able to provide a reduced fee based upon your ability to pay.
